You can now book appointments using on-line patient services. To register for this facility please call into the surgery for an application form or download it from this website's Online Services section. Alternatively, you can complete an application form online but you will still need to bring in two forms of ID.
You can choose any or all of the following on-line services:
- Book and cancel appointments
- Order repeat prescriptions (subject to review)
- Read your on-line medical records
You will need to provide two forms of identification: one with your photograph (i.e. passport or driving licence) and another confirming your address (i.e. a utility bill or bank statement). Owing to the sensitive nature of the information accessible, we cannot process the application without these two forms of identification.
If you want access to somebody else's records (for example, if you are a carer for somebody who lacks the capacity to use an on-line system) then we will consider an application for Proxy Access. You (and sometimes the patient, where able) will need to complete a Proxy Access application form, which is available from the surgery.
We strongly recommend that you read the attached information leaflet if you want access to your on-line medical notes.
Your doctor will be happy to give advice on the telephone when he/she is not consulting. The receptionists have been asked not to interrupt your doctor if he is with a Patient. You may therefore be asked to leave a telephone number for your doctor to contact you when he is free.
Please be aware that confidentiality issues will normally prevent the doctor from discussing the medical details of a patient with 3rd parties (even husbands, wives, etc.) unless prior consent has been obtained beforehand.
In a number of cases it might be worth considering an appointment with a practice nurse rather than a doctor. Practice nurses are qualified to deal with many conditions and you may be seen more quickly.
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else. The practice policy is that continuous non-attendee’s may be removed from the surgery list.
