If you reside within the boundaries of the SN3 3 or SN3 6 postcode, you are eligible to register at Eldene Surgery.
To register, ask at reception to complete a registration form (GMS1) together with a New Patient Questionnaire. This is so we can find out a little bit about your medical history until your medical records are transferred.
Those newly registered will be invited for an appointment with the medical staff.
Please note that at Eldene Surgery you are registered with the practice, rather than an individual doctor. You are thus able to choose and express a preference as to whom you see.
If you change your address or telephone number, please inform the receptionist as soon as possible. This helps to keep your records up to date and ensures that we can contact you for recalls and information.
Medical treatment is available from the date of registration. Please contact reception for further information.
If you are moving outside of the SN3 Postcode please do not use this website for your change of address as you will be moving outside the surgery catchment area and you will need to find a new GP Surgery. Many thanks.

Anyone in the above area, not registered at Eldene Surgery, who needs urgent treatment may be seen by the doctor as a temporary patient.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
